The network is a patient empowerment partnership between the University at Buffalo Department of Family Medicine, and patients from UBMD Family Medicine at Jefferson and Jericho Road Family Practice.

Morris Chair in Nephrology and chief of the division of Nephrology in the Department of Medicine in the University at Buffalo School of Medicine and Biomedical Sciences.
