CEAD WNY/ADMDC accepts referrals from physicians, hospitals, family members, and any individual concerned about possible memory loss and changes in cognition seeking a consultation.

How to Make an Appointment

To schedule an appointment, please call 716-829-5056 and a member of the team will respond to your request.

What to Expect During Your Visit

  • A new patient visit is approximately 1 hour.
  • We strongly recommend that a family member, caregiver, or individual that knows the patient well attends the appointment with the patient. Their presence plays an essential role during this initial evaluation.
  • Any previous testing, medical records, and scans (i.e., CT, MRI, or PET) should also be brought with the patient to the appointment.
