Policy Statement

Students diagnosed with Attention-Deficit / Hyperactivity Disorder (ADHD) who would like to have their stimulant medication refilled by a provider at Student Health Services must provide the office with adequate documentation of an ADHD diagnosis.

Diagnostic records and reports

Records must be:

  • Comprehensive, authoritative documents
  • Not more than three years old
  • Prepared by a licensed professional in psychiatry, neurology or psychology

Reports should:

  • Be on professional letterhead with the name, address, phone and professional credentials of each evaluator
  • Include date(s) of testing
  • Report all quantitative information in standard scores and/or percentiles

In addition, reports must be based on age norms.

ADHD Assessment Report

The ADHD assessment report must include:

  1. Formal testing of Attention
  2. Formal testing of Information Processing
  3. Formal testing of Aptitude/Cognitive Ability
  4. Assessment of Effort (Validity)
  5. Assessment of Academic Achievement/Adequate Preparation for College

Documentation from Others

We cannot accept documentation from family practitioners or school psychologists, as they are not ordinarily qualified to conduct the required assessments. 

Prescribing Stimulant Medications

The prescribing of stimulant medications is at the sole discretion of the providers at Student Health Services. We reserve the right to decline to prescribe stimulant medications should we feel the diagnosis has not been established, the student needs more specialized treatment or there are adherence and/or abuse concerns.

All records and reports should be faxed to 716-829-2564 in advance of your appointment.

  • Fax this information to Student Health Services in advance of your appointment
