Town Square for Aging Evaluation Planning Grant

The Town Square for Aging is an innovative community based intervention which is the first of its kind anywhere in the world. Located on the Weinberg Campus in Amherst the Town Square is facilitating the co-location and integration of health and social services for aging residents of Amherst New York. The Institute for Person-Centered Care has been engaged to help plan the evaluation of the Town Square which will incorporate research in to the impact on health, quality of life, ability to age in place, as well as evaluation of service quality.
