UB Seminars are controlled enrollment courses. As such, any attempt to repeat the course must be pursued in a subsequent winter or summer term.
This form is to be used by undergraduate students who failed or resigned their 198 or 199 UB Seminar and are seeking to retake their UB Seminar in a fall or spring semester, due to an extenuating circumstance.
