  • Childbirth, Adoption & Foster Care
  • Critical Life Events (Dean approval required)
  • Faculty informs Chair → Chair sends memo to Dean → complete memo sent to VP Faculty Affairs
  • Special Circumstances i.e. research setback (VP Faculty Affairs approval required)
  • Faculty request to Chair → Chair sends recommendation to Dean → Dean recommendation to VP Faculty Affairs
