As you prepare for financial disclosure, these questions help determine your reportable obligations.

If you answer yes to the series of questions in any of the following categories, then you must mark 'yes' on your FCOI form.

Do you or your family work for an organization other than UB?

If YES: Could this work reasonably appear to be related to your institutional responsibilities?

If YES again, you need to disclose.

Do you or your family hold a management position with an organization other than UB?

If YES: Could this management position reasonably appear to be related to your institutional responsibilities?

If YES again, you need to disclose.

Do you or your immediate family receive income from or own stock in a publicly traded company?

If YES: Do your interests in the company reasonably appear to be related to your institutional responsibilities?

If YES: Does the combined value of your stock in and income from this company in the past 12 months exceed $5,000?

If YES again, you must disclose.

Did you or your immediate family receive income from a non-publicly traded entity in the past 12 months?

If YES: Does this income reasonably appear to be related to your institutional responsibilities?

If YES: Does the income exceed $5,000?

If YES again, you must disclose.

Do you or your family hold equity interest (stock) in a non-publicly traded entity?

If YES: Does your equity interest reasonably appear to be related to your institutional responsibilities?

If YES again, you must disclose.

Did you receive more than $5,000 in the last 12 months in income from intellectual property?

If YES: Is this income from an entity other than SUNY or the Research Foundation of SUNY?

If YES again, you must disclose.

In the past 12 months have your travel expenses been paid for on your behalf and not reimbursed directly to you?

If YES: Was this travel related to your institutional responsibilities?

If YES: Was the travel paid for by an organization that is not a government agency, institution of higher education, academic teaching hospital, medical center or research institute affiliated with a higher education institution?

If YES again, you must disclose.
