Radioactive Waste Disposal Request (RW-50)

Request empty radioactive waste container delivery or full radioactive waste container pickup using this form.

Instructions

  1. If having a container(s) picked up, perform a container wipe test and attach raw data to the container. Radiation Safety staff will not pick up a container without the wipe survey attached.
  2. Submit form before 2:00 pm Tuesday for pickup on Thursday.
