Research Incident Reporting

Incidents sustained in a research setting must be reported to Environment, Health and Safety (EH&S) and Human Resources (HR).

Incidents that occur in a research setting must be reported. This includes cases where exposures may have occurred. Other examples include a needle stick, a spill or splash onto your lab coat or clothes, breathing in an aerosol or vapor that should have been contained, or a burn or shock from a lab procedure or faulty equipment. 

If an incident does occur, you must complete two forms:

  1. Research Incident Report
  2. Accident or Injury Report

Completing the Form

  1. Complete all required fields
  2. A member of the Environment, Health and Safety team will contact you within 2 business days.
  3. Remember to complete the Accident or Injury Report form to complete the reporting process.
