Secretarial Position Questionnaire

A supervisor must fill out this questionnaire for a secretary position and have the incumbent answer the required questions to obtain this secretary position through UB Jobs.

Completing the Microsoft Word Form

  1. Browse the Word document for instructions and form content
  2. Enter requested information into the form
  3. Print the form
  4. Get required signatures
  5. Scan a copy of the signed form for your records
  6. Send the original, completed, signed form by campus mail to the appropriate address:

To the supervisor:

  • requests for Secretary 1 or 2 positions must include the secretary position description questionnaire
  • the incumbent and/or item information should be entered into UB Jobs
  • the incumbent of the position should answer questions in section 1 and the supervisor should complete the questions in section 2
  • if program management and/or Human Resources have additional comments, they should attach to the completed questionnaire
  • attach the organizational chart for the unit where the incumbent works along with a program description
  • in the case of a new or vacant position, the supervisor should complete the questions in section 1

To the incumbent:

  • prepare your answers to these questions on the form; use additional sheets as necessary indicating the focus area of the questions
  • put your name, title, department and date on the first page and sign your name on the last page of section 1 certifying your answers are correct and complete to the best of your knowledge
  • give your answers to your supervisor for review, completion of section 2 and signature

Submit the completed questionnaire to the Division of Classifications and Compensation of Human Resources.
