Self-Identification of Ethnicity, Disability, and Veteran Status for Employees—Research Foundation (RF)

Research Foundation (RF) employees can specify their ethnicity, disability status, and veteran status. The self-identification forms are to be provided to newly appointed RF employees and, once completed, should be returned to the hiring unit for attachment to the RF new appointment transaction before submission to RF HR.

Completing/Submitting the PDF Form

For current RF employees direct submission

  1. Read through all pages of the document to find specific instructions, if any
  2. Enter requested information into the form
  3. Print the form
  4. Get required signatures
  5. Scan a copy of the signed form for your records
  6. Send the original, completed, signed form to:
