Completing the Microsoft Excel Form

  1. Request that IT vendor complete questionnaire in its entirety.
  2. Send the completed vendor questionnaire to the requestor OR if part of the Web-based Services and Software under $50,000 Pre-Purchase Form process, follow that form's instructions to submit the completed vendor questionnaire.
