W-8 Tax Form for Foreign Vendors (UB Version)

All foreign suppliers new to UB are required to submit a completed W-8 tax ID form and an Order Preferences, Sourcing & Contact form to Procurement Services as confirmation of their business identity.  This information is used to build supplier profiles in the three databases that support encumbrances from our electronic requisitioning system.  Scanned images of these forms will be included in the profiles.

Instructions

1. Complete the appropriate W-8 form

Complete the Order Preferences, Sourcing & Contact form located at the bottom of the W-8 instructions

 

2. Email the forms to the requestor
