If your UB phone was already in your work area or used by a former employee, you may need to contact IT staff to properly associate the phone with the desired UBITName.
The MAC address on your phone (if possible; see below for help finding this)
How to find your UB phone's MAC address
Cisco Unified IP Phone 7900 Series
Press the Settings key (checkmark icon)
Select Model Information (you may need to scroll down using your phone's arrow keys to find it)
The MAC Address is option 2
Cisco IP Phone 8800 Series
Press the Applications key (gear icon)
Select Phone Information
The MAC Address is displayed as the Host Name with SEP prefixing the MAC address
Cisco Unified IP Phone 7900 Series
Pick up your UB Phone and turn it around.
If you need to disconnect cables, make note of where each cable goes before unplugging: The cable from the wall goes to the connection with the yellow label marked SW. The cable from your computer and devices connects to the socked with the yellow label marked PC.
Look at the bottom of the device, below the connections.
A rectangular area with an embossed MAC indicator may contain a label with the 12-character MAC Address of your phone.
If the label isn't present, the MAC Address may be printed on the gray label in the center of the UB Phone's rear panel. If the stand is in the way, press the adjustment button on the side of the phone to release the stand.
If the MAC Address is not in either location, but your UB Phone powers on, place a service request and provide your location and an identifying labels from the connection at the wall. Technical staff may be able to locate the MAC Address from the networking equipment.
If the MAC Address is not in either location, and your UB Phone does not power on, provide as much information about the device as possible, including the model, telephone number (if known) and location.
Cisco IP Phone 8800 Series
Pick up your UB Phone and turn it around.

The cable from your computer and devices connects to the socked with the embossed symbol above representing a computer display and keyboard.
Look at the side of the rear panel, under the phone stand.
A black label is present with the device's serial number (SN) and MAC Address (MAC). The 12-character MAC Address is the correct information.
If the MAC Address cannot be found, but your UB Phone powers on, place a service request and provide your location and an identifying labels from the connection at the wall. Technical staff may be able to locate the MAC Address from the networking equipment.
If the MAC Address cannot be found, and your UB Phone does not power on, provide as much information about the device as possible, including the model, telephone number (if known) and location.
