Benefit Services coordinates faculty and staff requests for accommodation in accordance with the university's Reasonable Accommodation Policy. Follow the steps outlined below to request accommodation.
Contact Benefit Services to request your accommodation. We request that you send an email communication and include:
Benefit Services will review your request and may reach out to you for more information and/or medical documentation relating to your request. If medical documentation is requested, then the documentation should include the following information only as it relates to your request:
After Benefit Services has received all the information needed regarding your request, we will begin the interactive process, including your department, to determine if the request is reasonable.
Our office will work with you and your department to address your need for accommodation in a timely manner. However, some accommodations, including those relating to assistive equipment and or software may take additional time to address.
Please send any medical documentation, if requested, directly to Benefit Services. Do not send your medical documentation to your department.
If Benefit Services, in conjunction with your department, determines that your request is reasonable then we will send an email communication to you and your supervisor indicating that your accommodation has been approved. If your accommodation is denied in whole or in part, you will receive an email communication with the reason for denial and how you can appeal the decision.
