Yes, you will receive frequent reminders during the open waive/enrollment period to your UB email.
International waivers can be completed by visiting the Haylor, Freyer and Coon website. Please visit the International page for instructions.
A waiver must be submitted annually during the open waiver and enrollment period.
You will receive an email from notifications@uhcsr.com that an ID card is available for you, and to view, download or request a hard copy ID Card, to please visit the UHCSR website.
Log in to the insurance portal to look up participating providers or contact the insurance company.
For domestic students, please contact Haylor, Freyer and Coon by email to begin the enrollment process of adding your dependent(s) to your current coverage plan. Once enrollment is finalized, the insurance fee will be added to your student account bill.
For international students, please complete the dependent enrollment form and mail with check or money order to the insurance company.

Please contact Haylor, Freyer and Coon by email with a copy of your current insurance carrier's termination letter to begin the enrollment process. Once enrollment is finalized, the insurance fee will be posted on your student account bill.
To complete an enrollment with loss of coverage, please complete the enrollment form available on our website and provide a letter from your previous insurance showing the last day of coverage.
Yes, dental coverage is included in the 24-25 international insurance plan.

Certain situations do not apply as UB has already selected the most optimal plan for our students and the premium is billed and paid through student accounts. These premiums are billed annually in the fall for our Domestic students, and bi-annually ( fall & spring) for our International students.
