The University at Buffalo Information Technology (UBIT) operates as a covered entity as defined by the U.S. Department of Health and Human Services Office of Civil Rights. HIPAA Regulation Text 45 CFR Part 164.310(a) requires a covered entity to implement policies and procedures to limit physical access to its electronic information systems and the facility or facilities in which they are housed, while ensuring that properly authorized access is allowed.
UBIT implements policies and procedures to control and validate a person's physical access to highly sensitive and sensitive facilities that house ePHI Systems. Access is based on the person’s role or function. This includes visitor control and control of access to software programs for testing and revision.
This policy applies to all UBIT workforce members.
Workforce members: Adhere to all policies and procedures as written.
HIPAA Security and Privacy Officer: Controls and validates a person's access to IT facilities based on their role or function, including visitor control, and controls access to software programs for testing and revision.
Authorized Managers: Controls and validates a person's access to facilities based on their role or function, including visitor control.
Compliance Officer: Participates in ensuring that the security of ePHI is effective and enforced, in conjunction with the HIPAA Security and Privacy Officer and the Director of Facilities.
